Family Services
Our free Family Service programming works with the whole family by providing family case management based on the ACT model. The Assertive Community Treatment (ACT) model is an evidence-based practice that improves outcomes for families and individuals through a multidisciplinary team approach with consistent, caring and person-centered relationships at the forefront of all that we do.
Family Services Programs
Youth based Family Support
We work with youth in grades K-12 in out-of-school time programs that are play-based and movement based. This program provides parents with a safe, caring option for childcare after school and on summer breaks. We work to integrate the whole family into our programming through family nights, family education options and open communication at all times. Our youth and families are provided with mental health professionals onsite daily, mentorship and support, positive life shaping experiences, and all types of skill building including emotional regulation.
Family Intervention and Support
Our family intervention specialist works with families to increase protective factors through a strengths-based approach. We do so by connecting them to necessary resources within the community that help the family to have an increased quality of life as a unit. We also work towards this improved quality of life through family education and a space that creates a sense of belonging for all family members.
Emergency Family Funds
How to Qualify:
-Have a child 18 and under in the home
-Have previously applied for LEAP
-Have a plan moving forward
-Complete the application online (below)
-Have all documentation attached to the application.
-Complete over-the-phone interview
*funds will only be released to Vendors/Service Providers
