The Collaborative Management Program brings together agencies designed to improve outcomes for at-risk children, youth, and families involved in multiple systems.
Archuleta County Collaborative Management Program
Archuleta County is proud to be one of over 40 counties participating in Colorado's Collaborative Management Program, established in 2004 through House Bill 1451. This statewide initiative supports youth and their families by coordinating comprehensive, community-based services.
Program Goals
The Collaborative Management Program is designed with two primary goals in mind:
Support for Youth and Families
CMP provides wrap-around care and facilitated problem-solving for youth and families experiencing challenging life events. Our community-driven approach ensures that families receive the necessary resources and support through collaborative efforts.
Identifying and Filling Service Gaps
We actively work with local stakeholders to identify gaps in services within the community. By doing so, we strive to build tailored solutions that address unmet needs and strengthen the overall support system in Archuleta County.
Elgibility
To qualify for CMP, referred youth must meet the following criteria:
Be between the ages of 0-21
Be a resident of Archuleta County
Be actively involved in at least two systems
(e.g., Department of Human Services, mental health services, school issues, probation, etc.)
Referral Process
If you or a family you know could benefit from CMP support, please follow these steps:
Download the CMP Referral/Authorization Packet.
Complete the packet with the necessary information.
Email the completed form to Adriana at cmp@pagosacommunityinitiative.org with "Family Referral" in the subject line.

Meet our CMP Coordinator
Adriana Medina
Archuleta County Collaborative Management Program (CMP) Coordinator
Adriana works at the Pagosa Community Initiative as the CMP Coordinator, where her primary focus is supporting youth and families involved in multiple systems.
Adriana aims to reduce further system involvement through effective communication between agencies, resource connections, and prevention strategies.
Originally from Buffalo, NY (GO BILLS!), Adriana has called Colorado home for the past 8 years. Outside of work, she enjoys hiking, camping, fishing, cooking, watching stand-up comedy, and creating art.
