 Archuleta CMP

Archuleta County is proud to be one of over 40 counties participating in Colorado's Collaborative Management Program, established in 2004 through House Bill 1451. This statewide initiative supports youth and their families by coordinating comprehensive, community-based services.

​Support for Youth and Families


CMP provides wrap-around care and facilitated problem-solving for youth and families experiencing challenging life events. Our community-driven approach ensures that families receive the necessary resources and support through collaborative efforts.

Identifying and Filling Service Gaps


We actively work with local stakeholders to identify gaps in services within the community. By doing so, we strive to build tailored solutions that address unmet needs and strengthen the overall support system in Archuleta County.

Eligibility

Age: 0-21

Location: Be a resident of Archuleta County

 

Services: Be actively involved in at least two systems

(e.g., Department of Human Services, mental health services, school issues, probation, etc.)

Referral Process:

If you or a family you know could benefit from CMP support, please follow these steps:

Download and complete the referral packet:
 

Email the completed form to cmp@pagosacommunityinitiative.org with "Family Referral" in the subject line.
